The New Patient Appointment Experience

  1. GET SCHEDULED

    • Your first telemedicine visit is 60 minutes and includes a comprehensive history, review of any past labs, imaging, and treatments, and time to create your individualized health plan together.

  2. COMPLETE NEW PATIENT INTAKE

    • After scheduling, you will receive an email with a link to complete our intake form, along with the demographics page where you can share your insurance information, if you are an Oregon resident.

  3. GATHER YOUR HEALTH INFORMATION

    • To help us provide the most informed care at your first visit, please forward any relevant records from your current health care team, such as:

      • Blood work and other lab results from within the past year. Please request these if you do not already have copies. 

      • Imaging from within the past 3 years, if it is relevant to your concerns.

      • Chart notes from the last 6 months if you have been seeing a provider regularly for the same or related concerns.

    • These can be sent securely through the messenger in your Jane App account, or request them to be faxed directly to us at (855) 461-3309.

  4. JOIN THE APPOINTMENT LINK EARLY

    • You will receive an email with your appointment link 30 minutes before your visit. Please log in a few minutes early so you can troubleshoot any audio or sound issues and make sure your internet connection is stable.
