GET SCHEDULED
Your first telemedicine visit is 60 minutes and includes a comprehensive history, review of any past labs, imaging, and treatments, and time to create your individualized health plan together.
COMPLETE NEW PATIENT INTAKE
After scheduling, you will receive an email with a link to complete our intake form, along with the demographics page where you can share your insurance information, if you are an Oregon resident.
GATHER YOUR HEALTH INFORMATION
To help us provide the most informed care at your first visit, please forward any relevant records from your current health care team, such as:
Blood work and other lab results from within the past year. Please request these if you do not already have copies.
Imaging from within the past 3 years, if it is relevant to your concerns.
Chart notes from the last 6 months if you have been seeing a provider regularly for the same or related concerns.
These can be sent securely through the messenger in your Jane App account, or request them to be faxed directly to us at (855) 461-3309.
JOIN THE APPOINTMENT LINK EARLY
You will receive an email with your appointment link 30 minutes before your visit. Please log in a few minutes early so you can troubleshoot any audio or sound issues and make sure your internet connection is stable.
